Download the Waivers

Important: The waiver package contains three forms: the standard Blue Grotto adult waiver, a waiver/release form specific to guided cave dives and the Blue Grotto Cave Statement of Understanding. If this package opens in your web browser window, right click, control-click or do a File--> Save As to download it to your computer. Re-open the file in Adobe Acrobat.

In Acrobat, you’ll be able to fill out the forms and sign them digitally (you won’t need to have the forms witnessed if you do this). Filling out the fields on the first page will automatically populate corresponding fields on subsequent pages. Remember to put your guide’s name at the top of page 3, and to sign each page individually.

When you have completed and signed the forms, save the document. Email the package back to your guide, and print a copy for yourself and bring it with you. Download now…

Watch the Videos

The first page of the waiver package has a box you must initial to signify that you have watched the Blue Grotto safety and orientation video. Do so now.

Next, watch the safety and orientation video for the cave itself.

The statement you sign before you dive the cave includes an acknowledgement that you have, in fact, watched both videos in their entirety. Your guide will be relying on the fact you have done so and are completely familiar with their contents.
